Medicare Patients: We are an approved Medicare facility. Therefore, we will bill Medicare according to their allowable rates.
As a Medicare Part B particpant, you are only responsible for the annual deductible, the 20 percent of the charges known as "co-insurance," and any non-covered services. A Medicare supplement insurance policy may cover these. However, keep in mind you are ultimately responsible for this portion of your charges.
Private Insurance Patients: As a courtesy we will bill all insurances, whether we are contracted or not. It is your responsibility to provide us with all the information necessary in order to bill your insurance. Since we have no control over your policy limits, filing of your insurance does not relieve you of resposibility for the full charges.
Contracted Insurances: We are contracted with many major insurance plans. If you are covered under any of the plans, we will bill your insurance under the contractual guidelines. If your coverage indicates a co-pay, deductible or any portion for you to pay, you will be expected to pay that amount on the day of service.
Self-Pay Patients: If you do not have health insurance coverage, or if your insurance company will not cover your services at Scottsdale Eye Sugery Center, you will be asked to pay for your surgery on date of service with a credit card or a check. You will receive monthly statements which will keep you informed as to the status of your claim. If there is a problem, we will be happy to help you. We deal with a wide variety of insurance plans and may be able to cut through the "red tape" to resolve most problems.
